Coastal Senior Healthcare is seeking a highly motivated, ethical Nurse Practitioner to provide primary care and chronic disease management for our patients within an ALF setting in the Bradenton-Sarasota-Venice areas.

Full Time/Part Time positions available (Monday-Friday daytime hours)

Minimal call responsibilities, and no hospital rounding

Position Responsibilities:

• Take patient health history, assessment, diagnose, and treat patients within an ALF setting
• Timely documentation of patient exam in EHR system (Athena)
• Communicates to patients and caregivers the treatment plan and goals
• Management of patient medications
• Communicates with patients, caregivers, nurses, facilities, and other providers as necessary to assure proper diagnosis and treatment
• Medicare Wellness Exams

Requirements:

• Masters of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)
• Board Certified from AANP or ANCC as a Geriatric, Adult, or Family Nurse Practitioner
• Advanced Practice Registered Nurse (APRN) certified
• Advanced Practice Registered Nurse (APRN) license for the State of Florida required
• Active BLS/CPR certification
• Federal DEA License/eligibility
• Malpractice/Liability insurance
• Adult-Gerontology training experience preferred
